Dive Gear Information. Please provide us the Age Height Weight and Shoe Size. This is required. Without these you cannot board the boat.

All participants MUST have been diving within 2 years of your planned dive.

All participants with dive dates GREATER than two years CANNOT DIVE unless they contact Pro-Diver at 808-875-4004 or 510-681-1078 to make special arrangements.

Divers cannot travel above 500 feet altitude within 24 hours of the event.

Diving experience: Please provide Date of most recent Dive, (approximately) Number of lifetime dives, Certification level. This is required. Without these you cannot board the boat.

Dive Gear Information: The AGE, HEIGHT, WEIGHT and SHOES SIZE of each participant must be provided in order to provide you with correct pre-sized dive gear.
